How to Request a Transcript

(All request must be in writing)

Previous Students / Graduates

Please include your full name, year of graduation, date of birth, and address where you would like the transcript mailed. (Include maiden name if applicable.)  Mail requests to the address below. If you prefer to pick up your transcript in the Student Services Center, please email requests to Shelby Thompson, Student Services Coordinator, prior to pick up. Current Students

Students may complete a Request for Transcript form in the Student Services Office. Mrs. Thompson will process your request. The form requires students to list how they would like it sent, either directly to the college or for pick up in our office. You may email your requests to Shelby Thompson or call (254) 854-2215, ext. 1029.
